Background: regardless of many papers dedicated to hyperuricemia (HU) effect on the course of psoriatic arthritis (PsA), available data are hazy, and the issue of interest is still underinvestigated.
Aim: to evaluate an effect of hyperuricemia (HU) on clinical features of PsA.
Materials and Methods: based on the rheumatology registry of St. Petersburg (2005-2020), two groups of patients (HU patients with PsA (n=131), and non-HU ones with PsA (n=130)) were formed and retrospectively analyzed. HU was recorded at a blood uric acid (UA) level of >360 mmol/l in ≥2 tests ≥1 month apart in the absence of gouty arthritis.
Results: the patients were comparable in age and gender. In non-HU patients, the onset of polyarthritis (variant 2) was reported at the age of 60.53±10.35, and those with oligoarthritis or monoarthritis of the joints of the lower extremities/spine (variant 1) demonstrated its onset at the age of 48.64±13.36 (p<0.001). Along with this, the onset of variant 1 was significantly more common (p<0.001) in men than in women (84% vs. 67.86%, respectively), and the variant 2 was more common in women than in men (32.14% vs. 16%, respectively). HU patients demonstrated higher incidence of scalp and nail lesions (41.54% vs. 26.71%; p<0.01). Although the patients received comparable therapy, HU subjects were more likely to receive leflunomide (10% vs. 3.05%, respectively).
Conclusions: in patients with PsA, HU is associated with an increased incidence of scalp and nail psoriasis. Both polyarthritic and monooligoarthritic variants of PsA onset are equally common in HU patients. They were more likely to receive leflunomide, and the fact is impliedly consistent with more severe course of PsA associated with HU.
