Osteoarthritis (OA) is a chronic progressive joint disease, presenting a serious medical and social problem. The disease is associated with increased mortality risk and requires therapy as early as possible. The article analyzes modern treatment tactics in OA proposed by experts of the European League Against Rheumatism (EULAR), the Osteoarthritis Research Society International (OARSI) and the European Society for Clinical and Economic Aspects of Osteoporosis, Osteoarthritis and Musculoskeletal Diseases (ESCEO). According to current guidelines, patient treatment should be carried out using a combination of non-pharmacological and pharmacological methods. ESCEO experts noted that currently non-drug interventions in OA are not sufficiently implemented in practical health care, and it is necessary to improve the organization of their implementation in the future. The arti cle discusses approaches to basic OA therapy, including symptomatic slow-acting drugs for osteoarthritis (SYSADOA), discusses the effectiveness and safety of paracetamol, non-steroidal anti-inflammatory drugs, intra-articular administration of hyaluronic acid and glucocorticosteroids, as well as pain relief in patients with severe pain syndrome. Total knee replacement is recommended only for patients with the last stage of OA. A step-by-step algorithm for prescribing therapy in OA is proposed, which will be useful for practitioners.

Keywords: osteoarthritis, modern therapy, guidelines, comorbidity, symptomatic slow-acting drugs, SYSADOA, chondroitin sulfate, Alflutop.
