Eye Microsurgery Complex named after S.N. Fyodorov, Volgograd Branch
Purpose: To evaluate the efficacy of drug treatment in patients with primary open-angle glaucoma and individual IOP.
Materials and methods: The efficacy of drug treatment was evaluated in 383 patients with primary open-angle glaucoma (528 eyes). The mean age was 65,6±0,23 years. 318 eyes presented with initial stage, 132 – with developed stage and 78 - with advanced stage. Randomization for medical treatment was performed after individual IOP achievement. The first group achieved individual IOP with monotherapy of travoprost 0.004% (186 patients, 224 eyes). 197 patients (304 eyes) of the 2nd group required the combination of drugs to achieve individual IOP. Tolerant and individual IOP was evaluated by eye hemodynamics assessed by means of ophthalmotonosphygmography.
Results: Mean tolerant IOP in 383 patients (528 eyes) was 16,5 ± 0,1 mm Hg. Medical treatment decreased IOP by 8.1 mmHg (from 22,6 ± 0,1 mm Hg to 14,5 ± 0,07 mmHg, 35,8%). Individual IOP achievement led to improvement of eye hemodynamics. Tolerant pressure determination and reduction of elevated IOP with individual IOP achievement led to stabilization of glaucomatous process in 97,2% of cases.
Conclusion: Parameters of ophthalmotonosphygmography are helpful to determine the values of tolerant and individual IOP and to evaluate the efficacy of antihypertensive treatment in patients with primary open-angle glaucoma. For visual functions stabilization IOP fluctuations should not exceed the tolerant pressure.
Key words: primary open-angle glaucoma, ophthalmotonosphygmography, tolerant and individual IOP.
