Aim: to differentiate indications for open and mini-invasive operations in biliary reconstruction surgery.
Patients and Methods: 76 patients were under observation from 1994 to 2017 for iatrogenic extrahepatic bile duct (EHBD) injuries and cicatricial strictures of ducts and biliodigestive anastomoses (BDA). Group 1 included 36 (47.36%) patients with iatrogenic damages; group 2 consisted of patients with strictures of the extrahepatic bile duct and BDA — 40 (52.64%).
Results: in 25 (69.4%) patients of the group 1 (n=36), EHBD injuries occurred after video-assisted laparoscopic cholecystectomy (VALCE), in 10 (27.8%) patients with laparotomic cholecystectomy and in 1 (2.8%) patient after cholecystectomy from mini-access.
In group 2 (n=40) with anastomotic strictures, there were 15 (37.5%) patients who had previously undergone Roux-en-Y hepaticojejunostomy at the Praderi drainage for iatrogenic duct injury during the VALCE; with EHBD strictures after early interventions on choledoch duct for choledocholithiasis — 22 (55%) patients, with Mirizzi syndrome after choledocholithotripsy — 3 (7.5%).
Balloon strictureplasty after reconstructive operations in patients of group 2 in 20 (50%) cases was performed after 9.11±3.1 months. Repeated antegrade balloon dilatation of BDA strictures was performed in 9 (22.5%) patients. With further follow-up after 10.75±5.7 months. 4 patients (10%) needed additional endobiliary correction of the BDA.
Conclusion: the most common cause of EHBD injury was inflammatory changes in the area of the hepatoduodenal ligament with the presence of pronounced adhesions. Reconstructive surgeries in the form of BDA applying with the use of drainage framework are an effective way to restore the bile duct while respecting the optimal timing (less than 4 days) from the onset of iatrogeny before surgery.

Key words: iatrogenic bile duct injury, anastomotic stricture, balloon dilatation.
