The cardiovascular pathology keeps a leading position among the reasons for disability and mortality worldwide. Ischemic heart disease (CHD) and chronic heart failure (CHF) have the most significant social and economic value in the cardiovascular continuum. The prevalence of IHD in our country is 13.5%, in the US - almost 2 times lower - 7%. In 2013 every 9th death certificate in the United States was associated with CHF. To date, when talking about the treatment of cardiovascular diseases (CVD), the clinician needs to assign the pathogenetic therapy in order not only to correct the patient's quality of life, but also to improve the prognosis, reduce the risk of cardiovascular mortality. This article shows the positions of current clinical recommendations for the diagnosis and treatment of CHF (ESC 2016, AHA 2016). The complex approach in the choice of pathogenetic therapy is reflected, in particular the use of one of the modern pharmacological preparations of eplerenone (Espiro (Polpharma, S.A., Poland)) in the treatment of cardiovascular pathology. The article discusses the efficacy and safety of using this drug in patients with a combination of ischemic heart disease and chronic heart failure.
Key words: ischemic heart disease, chronic heart failure, mineralocorticoid receptor antagonists, eplerenone, espiro.
