Background. The paper addresses prevalence, mechanisms of chronicity and persistence of human papillomavirus (HPV) infection, modern diagnostic and treatment approaches to HPV-induced cervical dysplastic diseases. Special attention is paid to unresolved problems of residual damage of epithelium, relapse and risk of progression to malignancy. Possible ways of treatment, including modern antiviral therapy, are discussed as well as patients compliance to doctors recommendations. Results of comparative trial of original drug Inosine pranobex (Izoprinozin) in combined treatment of patients with cervical intraepithelial neoplasia are presented.
Objective. To evaluate the efficacy of CIN I treatment on the basis of short- and long-term results and patients compliance to doctors recommendations.
Material and methods. The study involved 62 women of reproductive age with dysplastic cervical diseases, highly oncogenic HPV genotypes and indication to destructive interventions. All patients underwent argon-plasma coagulation of the cervix, and were prescribed Izoprinozin 2 weeks prior to ablation within 10-14 days. Cytologic, colposcopic, virological controls and patient survey were used.
Results. Antiviral treatment with Izoprinozin was associated with good tolerability profile, adequate elimination of pathogens and complete epithelialization of the cervix after ablation, decrease of relapses frequency in case of compliance to treatment regimen. It is important to inform patients about transmission features, disease flow and possible outcomes of HPV infection in order to increase compliance to recommended course of drug therapy.
Key words: cervical pathology, human papillomavirus, combined treatment of HPV, antiviral drugs, HPV relapse, compliance.
