According to WHO prognoses, by 2020 non-alcoholic fatty liver disease (NAFLD) will occupy the first place in the structure of liver diseases. Pandemic character of its prevalence today is proved by the fact that it is found in 37% of the patients who visit general practitioners and therapists. Cirrhosis at the last stage of NAFLD is the second most common cause of the liver transplantation after the cirrhoses of viral etiology. The death rate among these patients is significantly higher than the death rate of those who suffers from cirrhoses of other etiology, along with other transplantation complications, including sepsis, graft rejection and cardiovascular events. This literature review provides information on the pathogenesis of the disease, the importance of the gut microbiota, the relationship between NAFLD and cardiovascular diseases, type 2 diabetes. The article considers the phenotype of NAFLD in patients without concomitant obesity, as well as the concept of metabolic syndrome from the standpoint of local visceral fat depots. The data on the methods of correction is based on clinical guidelines of Russian Gastroenterological Association, Russian Scientific Liver Society, European Association for the Study of the Liver and American Association for the Study of Liver Diseases.
Key words: non-alcoholic fatty liver disease, cardiovascular diseases, insulin resistance, gut microbiome, diabetes, dyslipidemia.
