Gastroesophageal reflux disease (GERD) is widespread in the population due to the high incidence of its risk factors. Among these factors, epigenetic factors (excess body weight, obesity, smoking, the use of drugs that promote relaxation of the lower esophageal sphincter, hiatal hernia, etc.) play a significant role. The impact of these factors is primarily manifested through the development of persistent motor disorders at the esophagogastric junction. Furthermore, it is essential to consider the aggressive and protective factors of the esophageal mucosa when evaluating the risk of disease progression and categorizing patients at high risk for erosive reflux esophagitis and Barrett's esophagus in the general cohort of patients with GERD. A treatment approach based on risk factor modification and the use of combination therapy with a proton pump inhibitor and an esophageal protector has been demonstrated to enhance the efficacy of GERD treatment in a number of ways. Firstly, it has been shown to facilitate the achievement of clinical, endoscopic and histological remission. Secondly, it has been shown to prevent disease progression, particularly in patients at high risk for erosive reflux disease and Barrett's esophagus.

KEYWORDS: gastroesophageal reflux disease, reflux esophagitis, erosive gastroesophageal reflux disease, Barrett's esophagus, epithelium-protective therapy, esophagoprotector.
