Bronchial Asthma and diabetes mellitus: the challenge of cytokine regulation

Pavlov First Saint Petersburg State Medical University, St. Petersburg, Russian Federation

The combination of bronchial asthma (BA) and type 2 diabetes mellitus (T2DM) can currently be viewed from the perspective of comorbid pathology, in particular, pathology characterized by shared pathogenetic mechanisms. Available literature indicates that the association between BA and T2DM is bidirectional and independent. Specifically, patients with BA face an increased risk of developing T2DM, while patients with T2DM are conversely more likely to develop BA. Potential shared pathogenetic links for T2DM and BA include cytokine imbalance, oxidative stress, glycation, microangiopathy, and endothelial dysfunction. Subclinical inflammation, marked by an imbalance of pro- and anti-inflammatory cytokines, exerts both local and systemic effects, serving as a prerequisite for the development of various comorbid conditions. Glycation processes, and specifically the role of the receptor for advanced glycation end products (RAGE), are considered one of the pathophysiological mechanisms for complications not only within the context of T2DM pathogenesis but also as a foundation for the development of BA. Investigating the molecular basis of this pathogenesis is essential for the development of personalized diagnostic and therapeutic methods.

Keywords: bronchial asthma, diabetes mellitus, cytokines, glycation, inflammation, receptor for advanced glycation endproducts.
