Premature ejaculation (PE) is a common sexual disorder. The diagnosis of PE is based on the time of onset of ejaculation, the ability to control it and the emotional impact on the patient. The article describes in detail the issues of the diagnosis and treatment of PE based on the physiology of ejaculation. Currently, the method of choice for treating PE is pharmacotherapy. For this purpose, the selective serotonin reuptake inhibitors (SSRIs) are used, and first of all dapoxetine. A large amount of data indicates the effectiveness and safety of the drug. Dapoxetine has a good tolerability and no serious side effects. To date, Primaxetine® is the only available dapoxetine containing drug registered in Russia. Along with SSRIs, tramadol or preparations for topical anesthesia are prescribed. In case of combining PE with concomitant erectile dysfunction, inhibitors of phosphodiesterase type 5 are used. Behavioral strategies play an important role in the treatment of PE, they are often used in combination with the drug therapy.
