Contact granuloma of the larynx is a rare benign disease, characterized by a high tendency to relapse and a multifactorial etiology, with vocal apparatus overstrain typically considered the main cause. This article details a clinical case involving a 49-year-old male patient in a voice-speech profession. Examination revealed notable features of anatomical asymmetry of the vocal processes of the arytenoid cartilages, specifically dystopia of the right cartilage, alongside local changes in the posterior third of the left vocal fold. Objective data, including the results of video endoscopy and computed tomography (CT), clearly demonstrated the presence of a granuloma. Associated structural changes were also identified, such as erosion and osteosclerosis of the arytenoid cartilage. Crucially, the authors put forward a hypothesis concerning the significant role of anatomical asymmetry in the persistence and recurrence of granuloma. Confirmation of this hypothesis necessitates further research. The findings suggest that the data obtained could contribute to the development of new approaches to treatment, which should potentially encompass not only traditional surgical removal, but also the correction of biomechanical factors of the larynx.
