Retinal vein occlusion (RVO) is one of the leading causes of vision loss in patients in developed countries, second only to diabetic retinopathy in prevalence. The pathogenesis of RVO is based on a triad of mechanisms including vascular wall damage, localized blood flow impairment, and hypercoagulability. These alterations may develop under the influence of various systemic and local risk factors, either individually or in combination. This review focuses on the role of local factors in the development of RVO. Elevated intraocular pressure and glaucoma-associated structural changes are shown to contribute to the development of RVO. The role of local vascular alterations as a shared pathogenic mechanism in both glaucoma and RVO is discussed. The impact of vascular dysregulation on the development of localized circulatory disturbances in RVO is also reviewed. Reduced ocular perfusion pressure resulting from decreased systemic blood pressure or elevated intraocular pressure, particularly in the presence of vascular dysregulation, has been identified as an important risk factor for RVO. In addition, a narrow anterior chamber angle and shallow anterior chamber have been recognized as risk factors for central retinal vein occlusion.
Keywords: retinal vein occlusion, vascular dysregulation, ocular perfusion pressure, intraocular pressure, narrow anterior chamber angle, shallow anterior chamber.
