The article is devoted to the issues of somnology and sleep disorders treatment. Cognitive behavioral therapy for insomnia (CBT-I) allows completely eliminating or significantly alleviating chronic insomnia in approximately 80% of patients. The aim of this article is to motivate change in medical thinking stereotypes. The author has set himself a rather ambitious task — to convince readers to take a fresh look at the problem of chronic development formation and insomnia progression into a chronic form. Thousands of articles and dozens of monographs on this topic have already been published. The problem consists of less as the lack of information or evidence, and more as simplified thinking stereotypes and the lack of physicians attention to this topic. In chronic insomnia, frontline treatment should be CBT-I. If it is ineffective or unavailable, drug therapy is used. What is more, when a patient complains about lack of sleep or sleep disorder, it is necessary to provide an early and intensive acute insomnia treatment, which should include both CBT-I elements and properly selected drug therapy in order to prevent chronic insomnia, taking into account the current data.
