The paper reviews the experience with thioctic (alpha-lipoic, or lipoic) acid, a natural coenzyme of mitochondrial complex. Thioctic acid regulates aerobic energy production in a cell. This agent is beneficial in liver disorders (acute hepatitis A, chronic hepatitis, fatty liver disease, liver fibrosis and cirrhosis), toxic damage (metals, fungal toxins), hyperlipidemia, atherosclerosis, diabetic and alcoholic neuropathies. The paper addresses evidence-based data on the use of various formulations of thioctic acid. Oral thioctic acid preparations approved in Russia for liver disorders and intoxications as well as parenteral thioctic acid preparations approved in Russia for diabetic and alcoholic neuropathies are described and compared. Parenteral thioctic acid preparations have maximal bioavailability. A number of clinical trials (ALADIN I, ALADIN II, ALADIN III, ORPIL, NATHAN, DECAN, SYDNEY, and SYDNEY II) were performed with parenteral Thioctacid®600T and oral Thioctacid®HR. They demonstrated the efficacy and safety of thioctic acid. As a result, Thioctacid®600T became a reference preparation of thioctic acid.

Key words: thioctic acid, intoxication, polyneuropathy, reference preparation, Thioctacid®, alpha-lipoic acid, free radicals, antioxydant, high release form.
