Chronic venous insufficiency of lower extremities is a heterogeneous pathological condition. Varicose disease (VD), caused by a deficiency of perforating veins (PV), is one of the main causes of this condition. Endovenous laser ablation (EVLA) of inadequate PV is a minimally invasive method of eliminating of horizontal reflux.
Аim: evaluate efficacy of the endovenous laser ablation (EVLA) of the perforating veins (PV) in the treatment of varicose desease (VD) as an alternative to surgical treatment.
Patients and Methods: The study included 42 patients (51 PVs), of which 34 women and 8 men aged 31 to 74 years (mean age 45.8 years) with insufficiency of the PV (one or more), or in combination with reflux in trunk varicose veins. All patients underwent the EVLA of PV with a diode laser with a wavelength of 1470 nm on an outpatient basis. After the procedure, the patients were examined after 1 day, 1 week, 1,3,6,12 months.
Results: obliteration was achieved in 41 patients with no reflux during a duplex examination. The 1-st patient required a repeated ablation with a successful outcome, which was performed one month after unsuccessful obliteration. Ecchymosis was observed in 26, induration in 21, paresthesia in 17 patients. Surface burns, arterio-venous fistulas and deep vein thrombosis were not observed.
Conclusion: EVLA is a reliable, safe and effective method of eliminating abnormal reflux in the PV of a hip and a shin as with the VD and with recurrent of the VD. Further studies are required to evaluate long-term results of the EVLA of PV.
Key words: endovenous laser ablation, perforating vein, varicose disease, recurrent varicose disease.
