Nowadays, more and more attention is paid to elderly patients with heart failure (HF), since such geriatric problems as reduced mobility, cognitive impairment and a large number of concomitant diseases are common among this group of patients. In 15–74% of patients with HF, senile asthenia is observed — a syndrome reflecting vulnerability to the effects of various stress factors and a decrease in the body's physiological reserve. Senile asthenia in patients with HF is associated with a high incidence of emergency hospitalization and mortality.
The presence of senile asthenia must be considered during risk stratification when treating elderly patients with cardiovascular diseases and making therapeutic decisions.
Clinical management of patients should take into account the assessment of the patient's geriatric status, non-cardiac comorbidity and their functional abilities.
Senile asthenia worsens the course of HF, increases the risk of decompensation, the need for urgent hospitalization and mortality. Timely detection of senile asthenia, competent medical care in accordance with existing guidelines, nursing and social support can positively affect the adherence to treatment, quality and life expectancy in this category of patients.

Key words: heart failure, comorbidity, elderly patients, senile asthenia, physiological reserve of the body.
