Coronary artery disease (CAD) is the most common noninfectious disease of great medical and social importance. Non-effective, non-rational treatment is responsible for high level of disability, morbidity and mortality. Myocardial ischemia is the result of imbalance between myocardial blood supply and oxygen demand. Imbalance between cell energy demands and capacity of energy production leads to development of myocardial hypoxia, accompanied by two cardinal pathological intracellular metabolism disorders: reduction of primary energy substrate production (ATP molecules) and activation of free radical oxidation processes leading to inhibition of cardiomyocytes functional activity and cell death. New perspective direction of CAD treatment is myocardial cytoprotection. Mechanism of action of modern cardioprotectors is based on energy exchange optimization, leading to decrease in myocardial oxygen demand, required for aerobic oxidation. Medicines are used to maintain and improve energy state of myocardial cells in ischemia and in case of depleted hemodynamic reserve.

Key words: coronary artery disease, myocardial hypoxia, antihypoxants, myocardial cytoprotection, lipid peroxidation, aerobic oxidation.
