The article presents the described clinical giardiasis case of the liver, where the following is reflected: possible errors and criticality in the diagnosis, the risks associated with this, and related specialists interaction. Diagnosis and treatment of an elderly patient complex condition, initially (presumably) associated with pneumonia, then with the metastases and a malignant neoplasm presence, is an example of successful interaction between specialists from regional and metropolitan medical institutions. The article shows the MRI image differences from metastatic lesions of the liver and hepatocellular cancer, measures to establish the correct diagnosis, increasing the criticality of the attending physician at the time of diagnosis, the involvement of related specialists, the relevance of the remote consultation system (“clinical telemedicine”). The images pr esented in the MRI dynamics are of the interest in terms of expanding the giardia foci understanding in the liver. They illustrate the treatment effectiveness. The article also presents the issues on clinical recommendations and specialists’ participation in the distance consultations system.

Keywords: liver abscess, metastasis, giardia, giardiasis of the liver, pneumonia, MRI.
