Granulomatosis with polyangiitis (GPA) is a systemic vasculitis, characterized by the development of granulomatous inflammation and necrotizing vasculitis of small vessels with a predominant lesion of the upper respiratory tract, lungs and kidneys. GPA remains one of the most severe and prognostically unfavorable forms of systemic vasculitis. In recent years, there has been a trend towards an increase in the number of patients with GPA. On the one hand, immunosuppressive treatment improves a long-term prognosis, on the other hand the incidence of the disease can really rise. The study of the role of cytokines in the development of the immunopathological process in patients with GPA is justified from a practical point of view in connection with the possibility of using genetically engineered biological preparations in this group of patients. The investigation of cytokines in relapse and remission GPA detects the potential target of the therapeutic intervention in case of exacerbation of the disease. Besides that, for the present time it is important to know the cytokine changes in generalized and localized GPA, since in the absence of treatment, more than 80% of patients with generalized form of GPA die within a year. In foreign literature there are conflicting data about immunopathological mechanisms and prevalence and activity validation methods in GPA patients. In native literature there are data about cytokines in ANCA- associated vasculitis (granulomatosis with polyangiitis (Wegener’s disease), microscopic polyangiitis, eosinophilic granulomatosis with polyangiitis)

Key words: cytokines, immune response, granulomatosis with polyangiitis, relapse, remission.
