Pregnancy in patients with abnormal collagen production and hemostasis disorders
Kudinova E.G.

Novosibirsk State University

Background. The paper addresses main features of hemostatic balance and functioning of uteroplacental complex in pregnant women, as well as interaction of hematopoiesis cell structures and connective tissue of the vascular wall.
Objective. To evaluate placental disorders and blood levels of minerals in patients with abnormal collagen production.
Material and methods. The study involved 625 pregnant women aged 18-24 years, divided into 4 groups. Group 1: 245 patients with menstrual disorders (MD) before pregnancy and abnormal collagen production (ACP), characterized by phenotypic and visceral symptoms of connective tissue dysplasia. Group 2: 115 patients with ACP without MD. Group 3: 200 patients without ACP and without MD. Group 4: 65 patients with MD without ACP.
Results. Patients with ACP are at the highest risk of pregnancy complications both for mother and fetus, due to disorders of uteroplacental and fetoplacental blood flow. Also they have decreased plasma levels of minerals (magnesium, calcium, zinc), as well as signs of endotheliopathy and hyperhomocysteinemia combined with genital tract infection. Increased thrombinemia due to activation of coagulation and fibrinogen pool separation was revealed in every third pregnant woman of groups 1 and 2, that was 1.8 and 1.3 times higher than in pregnant woman of groups 3 and 4.
Conclusions. Combined use of heparin, antiplatelet agents and preparations with magnesium ions, helps to normalize hemostasis parameters in pregnancy and improve condition of uteroplacental complex in pregnant women with ACP.

Key words: pregnancy, uteroplacental complex, hemostasis, collagen production, thrombinemia, homocysteine, menstrual disorders, magnesium, calcium, zinc, B vitamins, immune system, NK-cells.
