The review highlights histological characteristics of acute and chronic endometritis. Аim: to analyse scientific literature data describing histological manifestations of endometritis. A total of 245 sources were analysed. Open access papers from Google Scholar, PubMed, eLIBRARY, and CyberLeninka (search period: 2017-2025) were used for the study. Full-text articles, literature reviews, and original researches were selected for the analysis.

The review included data reporting histological features of acute (AE) and chronic endometritis (CE) considering etiology, morphotype, and CE course variant. Attention is paid to complicated pathomorphological identification of CE associated with high variability of morphological presentation in the endometrium. Also, the review is focused on differential diagnosis of specific and nonspecific chronic endometritis.

Although histological examination (HE) is a highly effective method to diagnose endometritis, it may demonstrate false results such as hypodiagnosis in case of incomplete morphological picture of chronic endometritis, and hyperdiagnosis in some cases of acute and chronic endometritis when subjective assessment of histological glandular-stromal endometrial characteristics is performed.

As a primary available method to diagnose endometrial condition, HE cannot always verify CE. Therefore, additional advanced methods of morphological diagnosis should be applied. However, routine HE remains an actual main method to assess endometrial compliance with a day of a menstrual cycle relative to a biopsy day, secretory transformation adequacy and presence of an inflammatory vascular component, as well as fibroplastic changes in the endometrium.

Keywords: endometritis, chronic endometritis, inflammatory endometrial disorder, endometritis pathomorphology, morphological diagnosis, endometritis diagnosis, histological examination, endometrial fibrosis.
