Acute bowel infections (ABIs) continue to represent a significant public health challenge. Predominantly, diarrheagenic viruses such as rotavirus and norovirus constitute the primary etiological agents in current clinical settings. Nevertheless, bacterial pathogens, including Salmonella, Campylobacter, Escherichia, Shigella, and Yersinia, remain significant contributors to bowel infections. The management of bacterial bowel infections necessitates the implementation of antibacterial therapy. Absolute indications for the initiation of such therapy include infections like dysentery, amebiasis, typhoid fever, and cholera. Antibiotics should also be used for bacterial ("invasive") ABIs that present with symptoms of colitis (notably including hemorrhagic colitis), and in severe or systemic instances of the disease, regardless of the patient's age or underlying conditions. Patients classified as high-risk also require antibiotic intervention regardless of age. Rifaximin has been identified as an effective therapeutic agent in treating bacterial ABIs, demonstrating efficacy in clinical research. The importance of a balanced method in prescribing antibacterial therapy, aligned with clinical guidelines, is underscored in the face of escalating antibiotic resistance among pathogens.

Keywords: bacterial bowel infections, diarrheagenic viruses, antibiotic resistance, rifaximin, diarrhea.
