Mastocytosis is a rare disease characterized by the accumulation of mast cells (MCs) in various organs, with indolent or aggressive flow.
Aim: To assess the quality of life and mental state of patients with different subtypes of mastocytosis.
Patients and Methods: Questioning and assessing the neurological symptoms of 30 mastocytosis patients aged from 20 to 66 years was performed. Patients were divided into the following groups: cutaneous mastocytosis (CM) (n=2), indolent systemic mastocytosis (ISM) (n=4), smoldering systemic mastocytosis (SSM) (n=3), aggressive systemic mastocytosis (ASM) (n=3), mastocytosis in the skin (n=18). The quality of life was assessed using a validated questionnaire to determine the Dermatology Life Quality Index (DLQI), and the Beck Depression Inventory (BDI) was used to estimate the psychoemotional status.
Results: in all patients with systemic mastocytosis, in contrast to cutaneous mastocytosis, a decrease in the quality of life was detected and correlated with the aggressiveness of the disease. Neurological symptoms, including headaches and cognitive disorders, were noted in 20 of 30 patients with mastocytosis (67%). In 12 of 30 patients (40%), signs of depression (BDI≥10) were diagnosed, and in 50% — moderate and severe. The mean BDI score for CM, ISM, SSM and ASM was 5±5, 12±5, 13±5, 32±4, respectively. Statistically significant differences were noted between the patients with benign forms (CM, ISM and SSM) and aggressive forms of mastocytosis (in this case ASM), in which 100% of patients suffered from severe depression (p<0.05).
Conclusion: The quality of life and mental status are mediated by the extent of internal organ damage caused by the underlying disease, and not by concomitant pathology and / or cosmetic defect. In patients with mastocytosis, there is a high risk of damage to the nervous system.
