Background: patients with purulent inflammatory skin diseases are prone to chronicity, complications, low efficacy of classic treatment, and high risk of long-term recurrence of skin lesions. These are the reason for searching for innovative diagnostic and therapeutic tools.

Aim: to study typical clinical immune metabolic disorders in patients with complicated deep pyoderma with the establishment of signal diagnostic formulas to improve the efficacy of basic treatment using additional immune metabolic therapy.

Patients and Methods: 125 patients with deep pyoderma associated with allergic dermatitis and true eczema before and after treatment and 30 age- and gender-matched healthy volunteers were examined. The clinical and laboratory examination panel used 7 clinical and 47 laboratory parameters grouped into clinical, inflammatory, immunological, and metabolic syndromes and their key components, as well as variational parametric and nonparametric statistical methods. In addition, standard diagnostic formulas for laboratory (blood-metabolic-immune) disorders and targets for differentiated immune metabolic effects on patients were calculated using the diagnostic relevance coefficient. The assessment was performed at baseline and 2–3 weeks after starting treatment in patients who received basic treatment and basic treatment plus a metabolic drug or immunomodulator.

Results: targeted correction of immune metabolic disorders in patients with purulent inflammatory skin diseases using metabolic drug and immunomodulator significantly improves the clinical and laboratory efficacy of basic treatment.

Conclusion: the pathogenesis-dependent induction of signal variations in laboratory status formalized in diagnostic formulas is demonstrated. The positive effect of additional immune metabolic therapy on clinical and laboratory parameters was established. An algorithm for using drug targets to select treatment options for complicated and uncomplicated deep pyoderma is described.

Keywords: allergic dermatitis, eczema, immune metabolic disorders, pyoderma, clinical laboratory efficacy.
