Polyarteritis nodosa (PAN) is a rare systemic necrotizing vasculitis characterized by the predominant involvement of medium- and small-caliber arteries, resulting in ischemic, inflammatory, and thrombotic changes affecting various organs and tissues. The disease is notable for its marked clinical polymorphism, which frequently leads to diagnostic difficulties, delayed diagnosis verification, and repeated revision of clinical hypotheses at different stages of patient follow-up.
This article presents a clinical case of a young patient with childhood-onset disease, a relapsing course, prominent cutaneous, articular, and neurological manifestations, as well as high laboratory inflammatory activity. A retrospective analysis was performed, encompassing anamnestic data, clinical examination findings, and laboratory and instrumental investigation results. Particular attention was given to the evolution of the clinical presentation, dynamics of inflammatory activity markers, development of complications, peripheral nervous system involvement, and response to the administered therapy.
The presented case illustrates the importance of comprehensive clinicopathological assessment, timely diagnostic verification, and early initiation of pathogenetic immunosuppressive therapy. Current approaches to the diagnosis, treatment, prognostic evaluation, relapse prevention, and multidisciplinary management of patients with polyarteritis nodosa are discussed.
Keywords: polyarteritis nodosa, systemic vasculitis, necrotizing arteritis, cutaneous vasculitis, glucocorticosteroids, cyclophosphamide.
