Abstract

Uratе continuum: approaches to diagnosis and treatment

A.E. Pikhlak Russian University of Medicine, Moscow

This article presents approaches to diagnosis and treatment of the proposed concept concerning urate continuum. The article also substantiates the importance and efficacy of in-depth laboratory diagnostics based on the advanced technology at all stages of the uric acid continuum clinical development. When using it, it is possible to identify early disorders regarded the uric acid metabolism and excretion by the kidneys, and to assess the sequence of events during complex dynamic laboratory study. The diagnostic difficulties of detection and identifying tophi in patients with comorbid pathology are considered.

When discussing treatment issues, nutrition recommendations are evaluated from critical positions, providing for a sharp purine restricted diet with a significant increase in the sugar-containing products consumption. This diet definitely contributes to the development of diabetes mellitus in the group of older patients. It is proposed to use a diet that limits only excessive consumption of purine products while prescribing hypouricemic therapy with allopurinol. The expediency of early treatment initiation is substantiated, as well as allopurinol characteristics at the stages of the uric acid continuum clinical development are considered.

Keywords: uric acid, hyperuricemia, gout, tophi, urate continuum, allopurinol.
